Patient Eligibility Requirements
Patients at SFMC must meet the following requirements to qualify for services:
- No health insurance
- High deductible insurance (catastrophic coverage only)
All services offered by/at SFMC are free to our patients. Our Providers make every effort to prescribe necessary medications from the pharmacies “$4 list” in order for it to be affordable to our patients. In addition to medical care, if a patient meets the requirements to qualify for obtaining medication through MedAssist we are able, for some medications and on a limited basis, to provide a 30 day supply of medication for use until the MedAssist medication is received by the patient.
In order to qualify for MedAssist/Project Access and Salem Hospitals Charity Care, patients income cannot be more than 200% above the Federal poverty level. Patients must provide verification of income (e.g. income tax forms, pay stubs, etc.) to attach to their applications. Patients are responsible for filling out all applications completely and providing the necessary paperwork required to qualify for these three programs.
